Care at Home New Jersey (CAHNJ)

Care at Home New Jersey is a nonprofit  organization that provides resources to support the homebound and their families and caregivers. Our offerings include a web based resource directory, technology that helps our patients stay connected with loved ones, an online forum for caregivers, as well as twice monthly on-site caregiver support groups. Our staff will help our patients connect with others, and to find the services they need, whether it be housing, in-home care, medical services, or food and transportation. Care at Home New Jersey was initially created to help met the nonmedical needs of Vanguard Medical Group's Primary Care at Home team.
